The Facts	The Common Sense Solution

An individual visits a physician an average of 3 times per year.¹ The most common office visit CPT codes are: 99214 = $85, 99213 = $45, 99212 = $31.88	Plan provides for 5 physician office visits. Beech Street PPO network can be accessed to reduce price.
Over 18% of the population visit a physician each year for preventative care. A physician will require a diagnostic test (x-ray, lab work, etc.) more than 87% of the time. ¹	Plan provides a wellness benefit with annual diagnostic testing.
A drug is prescribed after a physician visit over 73% of the time.² More than 66% of the population will use a prescription drug in the next year.⁴ Average generic brands cost $21 and are prescribed 65% of the time; brands cost $126 and are prescribed 35% of the time. ⁸	Plan includes insured prescription plan with $10 co-pay based on generic drug formulary with annual benefit of $1000. Branded drugs are available at discount from average wholesale price.
6% of the population spend 1 night in hospital and less than 1% spend a night in the hospital on two separate occasions. ³ Average cost is $1552; with a PPO network the cost is $1352.⁵	Plan includes hospital benefit with 1st time occurrence rider for calendar year to meet 99% of needs.
31% of the population will visit an outpatient facility ⁴ with 6.2% requiring minor surgery. ⁷	Plan includes surgery and related anesthesia benefit to be applied to surgeries. Also can be discounted within PPO network.
9.3% of the population will have an accident or poisoning related event requiring medical treatment.³	Plan includes accident protection to cover most accident events.
